Before arriving at our medical facility for your exam, please make sure you bring the following:

  • A valid picture government ID (driver's license or passport, cannot be expired)
  • I-693 paperwork (download at uscis.gov)
  • Official shot record (without proof of immunizations, you must receive all required vaccines)

We recommend that you print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record, to your medical exam appointment. Please carefully fill out the paperwork accordingly, completing Part 1 of the I-693 form, but do not sign your name until our medical team instructs you to do so. This form will be used to document and detail the results of your immigration medical exam.
